Terviselahendusega liitumisvorm – Tartu Ülikooli töötajatele / Health Plan registration form – for University of Tartu employees

Confido Terviselahendus on vabatahtlik ravikindlustus, mis hüvitab terviseteenuste kulud.  

Käesoleva vormi eesmärk on kutsuda teid liituma Confido Terviselahendusega. Selleks palume teil esmalt registreeruda. Pärast registreerimist saadame teie e-mailile kutse Terviselahendusega liitumiseks koos võimalusega valida sobiv pakett ning teha kindlustusmakse. 

Selleks, et Terviselahendusega liituda, palume teil tutvuda kahe erineva paketiga:

Tervis 400 TÜ – paketi maksumus kindlustusperioodiks on 400 €, millest Tartu Ülikool hüvitab 200 €. 

• Esimene osamakse summas 200 € tuleb tasuda Terviselahendusega liitudes. Sellest 100 € kompenseerib Tartu Ülikool tervisetoetusest. 
• Teise osamakse arve summas 200 € edastatakse teile 01.07.2023. Sellest omakorda 100 € kompenseerib Tartu Ülikool. 

Tervis 200 TÜ – paketi maksumus kindlustusperioodiks on 200€, millest Tartu Ülikool hüvitab 200€. 

• Esimene osamakse summas 100 € tuleb tasuda Terviselahendusega liitudes. Sellest 100 € kompenseerib Tartu Ülikool tervisetoetusest. 
• Teise osamakse arve summas 100 € edastatakse teile 01.07.2023. Sellest omakorda 100 € kompenseerib Tartu Ülikool. 

Kindlustusperiood on 01.04 – 31.12.2023.

Confido Health Plan is a voluntary health insurance that reimburses the costs of health services.

The purpose of this form is to invite you to join Confido Health Plan. We ask you to register first. After registration, we will send you an e-mail invitation to join the Health Plan with the opportunity to choose a suitable package and make an insurance payment.

In order to join the Health Plan, please familiarize yourself with two different packages:

Health 400 TÜ– the cost of the package for the insurance period is 400 €, of which the University of Tartu reimburses 200 €.

• The first installment in the amount of 200 € must be paid when joining the Health Plan. Of this, €100 will be compensated by the University of Tartu from the health allowance.
• The invoice for the second installment in the amount of 200 € will be sent to you on 01.07.2023. Of this, 100 € will be compensated by the University of Tartu.

Health 200 TÜ– the cost of the package for the insurance period is 200 €, of which the University of Tartu reimburses 200 €.

• The first installment in the amount of 100 € must be paid when joining the Health Plan. Of this, 100 € will be compensated by the University of Tartu from the health allowance.
• The invoice for the second installment in the amount of 100 € will be sent to you on 01.07.2023. Of this, 100 € will be compensated by the University of Tartu.

The insurance period is 01.04 - 31.12.2023.
