Look for appointment timesBladder neck and urethral stricture, i.e. narrowing, is generally related to previous surgery (TURP, TURB, radical or partial removal of the prostate) or prostate sclerosis and trauma, one of the causes of which can be acute or chronic inflammation of the prostate.
Narrowing of the bladder neck and urethra is caused by a strong scarring process, as a result of which bladder emptying disorders develop. In more severe cases, complete retention (urinary retention), bladder stones or recurrent urinary tract infections (UTIs) may occur. Modern studies have identified trauma, various instrumental procedures, previous bladder neck surgery and previous prostate surgery and/or radiation as the main causes of this condition. Inflammatory strictures are usually associated with the disease called lichen sclerosis, which is generally not manageable by endoscopic treatment. The incidence of bladder neck stricture is up to 600 cases per 100,000 people.
A symptom of a urethral stricture is a change in the normal stream of urine. Depending on the location of the stricture, the stream of urine can be thin and strong or thick, but sluggish and with a small arch. A double stream of urine may occur. Periodically, urine may come out in drops. A characteristic feature is post-dribbling, i.e. the dripping of urine after the end of urination, due to retention of urine in the enlarged part after the stricture. Chronic prostatitis, i.e. inflammation of the prostate, caused by a stricture of the urethra can cause a constant feeling of discomfort in the area of the urethra.
The operation is a controlled alternative to urethral dilation. An endoscope is used to enter the urethra and an incision is made in the scar tissue of the urethra or bladder neck under visual control. When the necessary diameter of the stricture resection is reached, an indwelling bladder catheter is placed with the aim of maintaining the size of the opening and ensuring haemostasis.
